Informed Owner Consent

Your dog’s initial consultation starts with a detailed case history, asking you (the owner) about how we can help you and your dog. This includes any relevant history (intake form) and information from other professionals e.g your veterinarian. Observations are conducted to better understand your dog’s behaviour and to ensure that your dog is safe to train. A training plan will then be discussed; this may or may not include training at that time. Training consists of you interacting with your dog in a safe and ethical manner at all times.

Privacy and Sharing of Information

I certify that the above information is correct to my knowledge. I authorise Serenity Dog Training and its associated staff to collect my dog’s personal and medical information as documented above, as well as communicate with my family Vet as deemed necessary for my dog’s training plan. I also understand that my dog’s personal and medical information is confidential and will only be disclosed to third parties with my permission.

Cancellation Policy

Your appointment time is reserved for your dog. A late cancellation or missed visit leaves a hole in the trainers’ day that could have been filled by another client and their dog. As such, we require 24 hours notice for any cancellations or changes to your appointment. Clients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee.

Liability Waiver

1. I HEREBY WAIVE AND RELEASE, indemnify, hold harmless and forever discharge Serenity Dog Training, and its agents, employees, officers, directors,affiliates, successors and assigns, coaches, teachers and trustees of and from any and all claims, demands, debts, contracts, expenses, cause of action,lawsuits, damages, and liabilities, of every kind of nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in anyway related to my participation in any of the events or activities conducted by,on the premises, of, or for the benefit of, Serenity Dog Training, provided that this waive of liability does not apply to any acts of gross negligence, or intentional, wilful or wanton misconduct.
2. I understand that the activities, in which I will participate, can be dangerous and may cause serious or grievous injuries, including bodily injury,damage to personal property and/or death. On behalf of myself, my heirs,assigns, administrators, executors and next of kin, I waive all claims of damage,injuries and death sustained to me or my property, that I may have against the aforementioned release party to such activities, including claims in tort, contract,equity or otherwise.
3. I acknowledge, agree and represent that I understand the nature of Serenity Dog Training, I AM IN GOOD HEALTH and in PROPER PHYSICAL CONDITION to participate in such activities. I further agree and warrant that if at any time I believe conditions to be unsafe, I will IMMEDIATELY DISCONTINUE further participation in the activity.
4. By this Waiver, I assume any risk, and take full responsibility and waive and of personal injury; death, damage, or loss of personal property, associated with Serenity Dog Training, including but not limited to dog training and behaviour modification.
5. This WAIVER AND RELEASE contain the entire agreement between parties,and supersedes any prior written or oral agreements between them concerning the subject matter of this WAIVER AND RELEASE. The provisions of this WAIVER AND RELEASE may be waived, altered, amended or repealed, in whole or in part, only upon the prior written consent of all parties.
6. The provision of this WAIVER AND RELEASE will continue in full force and effect even after the termination of the activities conducted by, on the premises of, or for the benefit of Serenity Dog Training, whether by agreement, by operation of law, or otherwise.
7. I have read, understood and fully agree to the term of this WAIVER AND RELEASE. I understand and confirm that by clicking "I agree" to this WAIVER AND RELEASE I have given up considerable future legal rights. 8. I have clicked "I agree" freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated me. Clicking "I agree" is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law.

Photography Release Form

I hereby grant and authorize Serenity Dog Training the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or later discovered.This authorization shall continue indefinitely, unless I otherwise revoke this authorization in writing.I waive the right to inspect or approve any finished product in which my likeness appears, including written or electronic copy.I agree that I have been compensated for this use of my likeness or have otherwise agreed to this release without being compensated. I waive any right to royalties or other compensation arising or related to the use of the photograph. I understand and agree that these materials shall become the property of Serenity Dog Training and will not be returned. I hereby hold harmless and release Serenity Dog Training from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.
